Making a referral

At CHT, we take our unique approach to providing community, housing, and therapy and combine it with a distinct clinical model that allows us to welcome, nurture and support those experiencing severe mental health difficulties.


Our support is trauma-informed and based on a relational treatment model.


Who we help

At CHT, we work with and support individuals with a history of multiple and prolonged hospital admissions or placement breakdowns, who are often unresponsive to treatment, and who may have very complex needs and histories of trauma.


Our residents typically have a diagnosis of personality disorder or psychosis and experience the most debilitating mental health symptoms including self-harming, suicidal behaviour, social isolation, and lack of independent living skills.

Our services provide both an alternative to hospital admission or the ‘next step’ after hospital. We focus on developing the skills and confidence needed for independent living, giving people a chance to live happy, healthy lives.

Residents can join different services depending on their needs:

  • Mental Health Recovery Communities – High Support: providing intensive 24-hour support, these settings offer structure and routine for those experiencing severe mental health difficulties.
  • Mental Health Recovery Communities – Medium Support: providing less-intensive treatments and a community-focused model to support individuals experiencing poor mental health back into independence.
  • Semi Independent Move-on Flats: These self-contained flats provide those who do not require high levels of support a way of transitioning to full independence.


FAQ


Our aim is to make the referral and assessment process as efficient as possible. Once we have received the referral, and it has been reviewed clinically, an assessment will be arranged. The assessment is usually carried out by the service manager or psychotherapist, you will also be invited to visit the service on a separate occasion. We then write an assessment report which is shared with the care team and taken to a local authority or commissioner funding panel. It is important to note that factors such as more complex referrals and delays from funding panel decisions might affect the length of the assessment process.

How long does the referral and assessment process take?


Placements are typically funded through the individual’s local authority or the NHS.

How are placements funded?


Length of stay at our services varies, our residents have regular placement reviews with their care teams and our staff. The average length of stay is around two years.

We typically receive our referrals directly from hospital, Community Mental Health Teams, care coordinators and social workers.
